This 18-year-old female has been bothered by diplopia when looking “to the side” for the past year.  She has been treated with immuno-suppressant for 6 months.  Her thyroid profile was normal and her collagen vascular profile was negative.  There was no evidence of cysticercosis.  Motility in the left eye is limited as shown in the clinical pictures and the CT showed thickening of the left medial rectus.

1.  The likely diagnosis in this case is:  
 

a. thyroid myopathy
b. metastatic carcinoma
c. myositis
d.  a worm in the muscle
e. none of the above

2.  A diagnosis could be confirmed by finding:   
 

a. a normal tendon
b. inflammatory process extending into the tendon
c. an elevated eosinophil in the differential count of the white blood cells
d.  finding flare and cell in the anterior chamber with keratic precipitates
e. none of the above

3.  Treatment for this condition is:
 
a. systemic steroids
b. antihelmenthics
c. irradiation
d.  Botox
e. Azythromycin
