This six-year-old boy, who is otherwise in good health, presented as shown in the pictures above. The defects have been present since birth. There is no vision in the right eye; the left eye is normal in structure and function. Birth history was unremarkable except for the defects shown.

1. The most likely diagnosis in this case is:
