| A 38-year-old myopic male presents for a routine eye examination. He notes that he occasionally has blurred vision with halos during exercise. On exam, he has 20/25 vision in both eyes; however, his IOP is 29 mmHg in the right eye (OD) and 27 mmHg in the left eye (OS). His cup-to-disc ratio is 0.8 in both eyes. On gonioscopy, the above is seen. |
All of the following are associated with this condition except: |
|
d -- iris colobomas
This is a classic case of pigmentary glaucoma which has recently been linked to genes on chromosome 7. This type of glaucoma is caused by a reverse pupillary block mechanism where the iris bows against the zonules. This mechanical contact between the zonules and iris liberates pigment which can obstruct the trabecular meshwork. This is not associated with iris colobomas.
