This 10-year-old girl from India has 20/200 vision in straight ahead gaze.  Her vision improves to 20/100 with her chin down.

1.  This girl is likely to be found to have:   
 

e -- all of the above

This patient with such pale skin coming from India is likely to have albinism. Albinos will be expected to have nystagmus and a pale fundus due to lack of retinal pigment. Since she has better vision with her chin down and looking up, it is likely that her nystagmus is quieter in up gaze.

 

2.  Her condition would be most likely:  
 

d -- recessive

Albinism is a recessive trait.

 

3.  Treatment for the head position could consist of:
 

b -- recession of the superior recti and resection of the inferior recti

Since the null is in up gaze, surgery to restore normal head position would strive to mimic the innervation needed to quiet the eyes.  Putting the eyes “down” with bilateral superior rectus recession and bilateral inferior rectus resection would cause the same innervation for primary position gaze after surgery as would be required to have the eyes look up for a quieter position pre-operatively.
