QOW49

A five-year-old boy was presented for an examination after failing a screening test for vision. His visual acuity was measured at OD 20/200 and OS 20/20. His eyes were aligned in the primary position, but on cover testing an intermittent esotropia of 15 prism diopters was measured. After cover testing, his eyes recovered alignment promptly. Stereopsis was measured at 4000 seconds (theTitmus fly). Cycloplegic refraction was OD + 5.75 and OS + 2.00. The remainder of the eye examination was within normal limits.

1. The diagnosis in this case is:

a. ammetropic amblyopia
b. strabismic amblyopia
c. congenital amblyopia
d. anisometropic amblyopia
e. none of the above

2. Glasses should be prescribed as follows:

a. full cycloplegic refraction
b. full cycloplegic refraction in the right eye and as much plus as the boy will tolerate in the left eye
c. reduce the plus 25% for each
d. prescribe as much plus as the boy will tolerate in the left eye and reduce the plus in the right eye by the same amount (if any) in the right eye
e. none of the above

3. Amblyopia treatment should be carried out as follows:
a. patch the left eye full time until vision is equal
b. patch the left eye half time
c. put cycloplegic drops in the left eye to produce fogging and induce use of the right eye
d. provide glasses as determined according to the answer for question 2 with no patching now and follow vision
e. none of the above
