This 21-year-old woman presents for examination after receiving immediate care for injuries received in a motor vehicle accident where she sustained facial trauma.  She now has double vision, enophthalmos and limitation of movement in the right eye.

1.  Work up should include:   
 

a. determination of visual acuity
b. thorough eye examination
c. testing of sensation of the cheek comparing the right and the left
d.  imaging of the orbit
e. all of the above

2.  The most likely diagnosis is:  
 

a. fourth nerve palsy
