Strabismus - Vertical Diplopia After Cataract Surgery

History: This patient is a 79-year-old man. He is in good general health and is taking no medications. He underwent uncomplicated cataract extraction of the left eye fourteen months ago. Six months ago he had cataract of the right eye. Local anesthesia was used in both procedures. When the right eye was done, three additional parabulbar injections of local anesthesia were required. Immediately after the second procedure the patient complained of vertical diplopia. Twelve diopters of left hypertropia (right hypotropia) is measured in the primary position. The patient has 20/20 vision in each eye with spectacle correction for low myopia.
