Strabismus -  "Y" Pattern Exotropia Lecture 19 of 38  NEXT»

HistoryThis four-year-old boy has had strabismus since birth. He has a small exotropia in the primary position measuring 20 prism diopters.  The exodeviation in up gaze is very large and hard to measure, but is greater than 90 prism diopters.  In down gaze, the eyes are aligned.  In lateroversions there is slight elevation in adduction in right gaze and slightly more elevation in adduction in left gaze.  This appears to be a "Y" pattern exotropia.  The child appears to see well with either eye and has a cycloplegic refraction of right eye +1.00 and left eye +1.25.  The media are clear and the fundus is normal in each eye.  The child is otherwise normal and healthy and there is no family history of strabismus.  
 
Diagnosis:  "Y" pattern exotropia
